I use Ilium's eWallet, since I keep everything else in there, as well. Not free, however, unless you only have 5 passwords. :) Then you can use their 'freewallet' app.
However, if you want to document your credit cards, ssn's, and insurance policies, in addition to the passwords (with links to the websites) it's a pretty good program.
