You get the work/internet choice window by tapping on the Settings button when WiFi is not turned on. If it is on, then it probably still relates to the problem you mentioned in another thread and sounds to me like a hardware problem.
The only other possibility I can see is that some other software you've installed is causing the WiFi adapter failure, probably by overwriting some driver files. That would be a difficult issue to problem solve by anything less than a hard reset. Of course, after the reset, restoring a backup wold probably restore the problem and simply reinstalling all the software you are using would likely add back the problem, too, so you'd probably need to install one at a time until the problem resurfaced.
