I see that sort of thing on a fairly regular basis with MS Reader. It's more of a reader or an OS issue of some sort with memory cards. I have it happen with both a 512mb card and a 1gb card I have. Don't even have a 2gb card.
For me it is more that it looses connection in the middle of reading a book. Especially when I've been reading intermittently on breaks and lunchs at work, but will even happen when I'm reading a book straight through. Will hit a spot where the line just stops in the middle of the page and its not the end of a chapter. If I try to push on I get the message about reader not being able to access the book anymore. But I've also seen it happen the way you describe. Happened a few days ago in fact, and I wasn't even messing with my library really.
What I usually do is jump out of the book to the library (if I'm in a book when it happens), go to the Memory settings and stop MS Reader. Open reader again fresh, it finds the books, I open the one I was reading and everything is ok. If that doesn't help, and that is rare, I either reset the PDA, or hunt down the annotations file for the book, delete it and let Reader repaginate the book.
Now, I've found this is something that only happens to books on expansion cards. I used to do things like transfer books from my SD card to main memory when I wanted to read them, but its about as much effort as leaving them on the card and shutting down the app when I need to.
