I went and found the document from HP I mentioned so here some more info from it. I found it last July so some of it maybe dated but I have used it to solve several quiirky USB problems. It doesn't have an ID number but the title is
"HP LaserJet Series Printers - Troubleshooting USB Chipset Issues"
It's 17 pages of valuable USB troubleshooting techniques
Affected Chipsets covered
Intel 82371, 82801
VIA prior to April 2000 and same vintage ALI
AMD 756 Chipset
SIS 7001 USB Host Controller
They do mention that in many cases a powered USB hub will correct some problems
