Try ThemeBuilder 2003, it's free. Create pictures with the specs listed here:

Alternatively, you can try to get SPB's Extended Theme Generator to work; in my experience, it does not do anything at all, and the Microsoft Theme Generator 2002 remains unchanged. Good luck.
