I finally figured it out, aka stumbled into to the solution. The link existed in two places, in \windows\start menu and in \windows\start menu\programs. I deleted the link in \windows\start menu and the problem went away, i.e. only one entry on the pick list and no more error message when I clicked on it. :approve:

1. Get rid of the dups on the list
2. Fix the error so I can uncheck the app
