I am looking to make a template that allows a moblie user to check off job site tasks i.e. Lighting working (check yes/no), proper labels applied (check yes/no), etc. I tried to write a word template but it has formatting issues when put an Axim/PocketPC equivalent device. Any freeware would be nice, but we are just looking at anything that will get the job done.

Also I need an app that can handle playing .wav files that we have for voice mail. WM Player can't play these files (eventhough Cisco says that the sound format is compatible with Windows Mobile 2003). Any help there would also be appreciated

Try XSForms and XSDesigner. Both are free and allow you to create forms like you describe and when you create the form in XSForms, the app automatically generates an XML database to store the results. You can then import the data into almost any program.
