I am looking for a program that allows me to create a personal record of the medical history of myself as well as members of my family. The data should ideally include:

1. Date of appointment;
2. Doctor's name;
3. Diagnosis;
4. Medication;
5. Result of treatment;
6. Etc

Any suggestions?

There is a program, now free, which physicians use, to track their patients:

Patient Tracker for Pocket PC

I think you could use this program to tract health data for yourself and family members.

Thank for you link but the free version is no longer available. There is a version 5.2 available. I have downloaded bunch of trials and will post which one I like and why after I have tested all of them.
