I feel stupid but ... what are they? And what's the difference?

Themes are designs used on your Today screen, to give your device a personal look and added functionality.

Skins usually refer to a different design and/or interface used by certain programs for the same reasons.

Your Today screen can actually have both... a Theme you design as a background, and then applications like Wisbar/Icbar/etc are "skinned" as well as a separate operation to complement Today or to add extra functionality.
