My 256MB Lexar SD card, AR0306LR just asked me to format, and works for a few days, then did it again, today I sent in the RMA form to Lexar, Luckily I still have a verbatim and a panasonic SD card to fall back on, which are made in japan

Bradenpro: what is the code and country on the back, also, on the front, is it just one big sticker saying lexar 256 blah blah blah, high speed media or is there a sticker on the bottom 3/4 of the card and SD logo physically printed on the upper 1/4 of the card

Yep, I specifically requested a card made in Japan on the RMA form, and I also said "I feel misled when your packaging indicates the product is manufactured in Japan, but on the product itself, it is indicated that it is made in china", which should help rub it in.

i had the same damn card. Luckly lexar has great support. i did it in live chat and my card came after like a week. It was a japan one, lexar only sends the japan cards now so you shouldnt have a problem.
I saw on another forum that was talking about this that if you format the card under windows xp they said their problems went away and haven't come back, might be worth a try??? I've had several lexar cards only one went bad. I got on the chat and they replaced it no hassles.
I have tried formatting in windows XP, first time around, I formatted using all the default options when presented with the format dialog when using the card with my USB card reader, card failed after two days, formatted again in fat16 using the command prompt, card faild after transferring 100MB of MP3s, card is now unstable and works extremly slow for possibly an hour of use before needing to format. It was being used mainly for games and MP3s so I didn't lose too much, but I had a few personal docs and two apps that got lost because I didnt back up the SD card, nonetheless, everything important is on my 256MB panasonic card, I am just short some music now :D
RMA form received today from lexar, I have the address and RMA number now, and will be shipping it out tomorrow.
--I really dont think it matters what OS you use to format it, it depends on what file system is used, I have also heard that formatting in FAT32 will get some extra out of the card, but I have no reason to get a few days more out of it, I want to get the RMA over with.
The problem is definetly the axim, whenever I access files with that is when I have issues, I could do the happycheesecake tests, (all of them, not just 1 and 2) with my USB card reader and have no issues with the card. I am very displeased with my lexar product in the axim, but not displeased with the product overall, and their tech support is great, and I have no reason to not buy from lexar in the future, its support like this that keeps customers.

I have a Lexar 256MB (made in Japan) and sometimes it is not recognized from the PDA and also from my Dazzle Reader.
After removing and inserting again it was always accepted up to now.
I have never reformatted it. Do you think the card is on risk to fail completely?
How can I get a replacement, I bought it on Ebay from a seller in America?
Since it is a japan lexar card, and those are one of the best SD cards money can buy, I dont think it will fail completely, If I were you, I'd format the card (you can do it with your card reader, and in windows, right-click on the drive that your card reader makes your SD card, and click format, select FAT, and click format)
Then, put the card back in your axim, I dont think you will have any problems with it. Also, blow in the SD slot, just to clear out any dust or anything that may have gotten in there (make sure not to spit in it :p)
As well, make sure the LOCK switch on your SD card is up and not down (as in unlocked and not locked)
