Just uninstall your present version and download the zip file (cab version ) from here. http://www.freewareppc.com/multimedia/notem.shtml.
If you download using your PC, extract the cab file (unzip), transfer it to your axim and run it directly on your axim.
If you have Resco file explorer and internet on your Axim, download it directy to your Axim and simply tap the cab file. Install on default location.

This copy of noteM works for me. You don't have to be on A12, ROM versions don't matter.

Uninstall the application. Use a program such as memmaid to clean out the registry and generally clean the axim of unused files. Dinarsoft has a trial version of the program. Soft reset several times and run the cleaning ap again, just to make sure you have everything cleaned up. Reinstall the program. Just as a note, I have mine installed on an SD card and it works fine.

Just another note, while it also works when you install it on SD it's really just a matter of preference. In my case, I use a different SD card to store all the lectures and seminars that I attend so that when someone needs to use the recordings, I can just give them that card with no hassles. If you run out of memory for whatever reason, you can just put another one in. When finished, I can keep the card in my library. That's why I recommend installing it in main memory. You can use a CF card if you decided to install it on SD with the same concept, but like I said it's just a matter of preference. I have another PDA w/only an SD/MMC slot so SD is my preference.

On a side note, that's also the reason Resco picture viewer is also installed in my main memory. So I can use it to view picture files from my camera SD right after picture taking.

I agree, having programs on one location or another is personal preference. I usually have critical apps on main and everything else on SD, and since my camera uses CF I have photo viewer on SD too.

Do you guys with X51(v)s and NoteM find that that there are two bugs in WM5?

One is with the recorded files list - all the text is messed up, and you can't see what you've recorded...
The second is that the "Disable Automatic Power Off" setting doesn't work - you have to manually disable it each time you record or it'll cut off at whatever you set your "Turn off device after x minutes" is set to.

The second one is really annoying =/
