The generic icons usually indicate that the file no longer exists but the link is still there. Is the SD card properly seated in the slot? Make sure, then try another soft reset. BTW, which card are you using? Card failure is another possibility.

I am having this problem about every 1-2 weeks. I never remove my SD card, so it must either be a defective Axim where the card keeps coming loose or a defective card. How do I possibly find out which? Also, how come the program links remain broken even though I can access files on the card?

To test the card, try it in a card reader or another device that accepts SD cards. If you can access the programs, try a soft reset to get your icons back.

I don't have a card reader, but I can see everything on the card and even open some files with Resco Explorer. Soft resets don't restore the lost icons though.

I've tried two SD cards and everytime I do a soft reset, my icons disappear.
Also . . . whenever I power off the unit while I Avantgo is up and running (I am using the new standalone version), and then power back up, I get an Avantgo message that the card on which the Avantgo files are stored is missing. The program exits. I can restart it without a hitch.
Hmmm . . . sounds like my Axim is slow (or hesitates)reading the SD card. Whatdya think???
Failure Mode: This is a bug with Windows Mobile. After soft reset, storage cards are not detected / mounted prior to the start menu links being searched. Since the storage cards are not detected a default icon, usually associated with broken application links, is displayed in the start menu.
Tech Support Work Around (Inefficient): After soft-reset, select "Settings" from the Start Menu. Select "Menu" from the settings window. Uncheck and recheck any program. Select "OK" to exit the Menu window. This will force the PPC to re-scan icons for all applications. If the application is present on the storage card, correct icon will be associated and displayed.
Long Term Resolution (Professional League): TBD. Microsoft may recognize this bug and address it in a service pack. Another possibility is some "tweak" utility that forces a refresh of the \Windows\Start Menu\ folder after the storage cards are detected / mounted.

I remember seeing a registry setting that I believe fixed this item. It changed the default wait time before the PocketPC tries to access the image information for these icons during boot-up. But, I don't remember where I saw it. :( Apparently the problem is that with some slower SD or memory cards the PocketPC doesn't wait long enough for the card to respond back before starting up. So, all icons show up as a default image.
Sorry I can't find it but maybe if someone knows they could post it.

I found if I installed the program in the root directory of the SD or CF card and put a shortcut to it in my start menu or program menu the icons were there, even after a soft reset or hard reset.
I don't know how deep into the directory this fix will work, perhaps one level, two definitely didn't work for me, but in the root directory it works fine, just difficult to have your programs all in the root directory.
Will experiment further, for now working well.

This fixed the problem once I found a way to extract the Arm version from a .lzh file... Found a program called 'Alpha' which allowed a trial download. I moved the Arm version to the Axim "Windows/Start Up" file folder and it runs with every soft reset.
Thanks for the link and if anybody has a problem with the .lzh file, send me a Private Message and I'll forward the .exe to you.
Anybody has a good solution now? It's quite weird when they design the OS assume everybody won't install the programs on storage cards.

Noticed this after loading WM5, and as best that I can find it's a bug in the OS. I can go into settings/menu and deselect, then select any item and the icons reappear. I've tried 3 different cards and each one does the same thing. Just stuck for now I guess.
